Qualification:

  • A license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession; or
  • Master’s degree in a human service field and has one year of full-time, pre- or post-bachelor’s degree accumulated supervised MH/DD/SA experience with the population served; or
  • Bachelor’s degree in a human service field and two years of full-time, pre- or post-bachelor’s degree accumulated supervised MH/DD/SA experience with the population served

  • Can work efficiently and effectively within a team, and independently
  • Have effective oral and written communication skills
  • Case management proficiency
  • Expert in crisis intervention techniques

Job Responsibilities

  • Conduct initial assessments of clients with serious and persistent mental health problems and connect them to a higher level of care services
  • Develop Person Centered Plans and Crisis plans to meet each client’s needs
  • Observe and monitor the client’s response to treatment and make adjustments to treatment when necessary
  • Provide community-based therapy and case management to clients with mental health concerns
  • Inform clients about available community resources and how to access and benefit from them
  • Monitor the utilization of services provided to clients to ensure they are effective, appropriate, and within limits set in the treatment plan and service authorization
  • Track authorizations and secure reauthorization of services before they expire
